Hospital at Home Programs and Equity

Presenter: 

• Ab Brody, PhD, RN, FAAN, FPCN (Mathy Mezey Professor of Geriatric Nursing and Professor of Medicine; Associate Director, Hartford Institute for Geriatric Nursing, New York University) 

Summary:

In this talk Dr. Brody will first provide an overview of various hospital at home program models and their outcomes based on over two decades of research. He will then discuss implementation facilitators and barriers, the unique skill set required of teams caring for older adults in this model, and both the positive impact and challenges the model can present to health equity and caregiving.

Learning Objectives:

• Understand the advantages of hospital at home programs for older adults with acute conditions that can be safely treated at home.
• Describe the facilitators and barriers to implementation of hospital at home programs in varying geographic and socioeconomic environments.
• Evaluate the methods by which hospital at home can decrease inequities rather than widen them in disadvantaged and underserved populations

CAPABLE: An Interdisciplinary Approach to Aging in Place

Presenter:

• Amanda Goodenow, MS, OTR/L (Strategic Partnership Coordinator at the CAPABLE National Center) 

Summary:

CAPABLE is a person-directed, home-based program that addresses both function and healthcare expenses. The four to five-month program integrates services from an occupational therapist (OT), a registered nurse (RN), and a handy worker who work together with the older adult to set goals and direct action plans that change behaviors to improve health, independence, and safety.

Learning Objectives:

• Describe the evidence that supports CAPABLE and the urgent need to adopt models of care that fully integrate the social determinants of health along with the clinical status of the older adult.
• Describe the program components and team.
• Describe how CAPABLE addresses equity and promotes self-efficacy.

Resources for Aging in Place at Home

Presenter:

• Jeff Wiberg (CEO, Family Resource Home Care; Board President of the Home Care Association of America)   

Summary: 

This session will explore some of the data and resources illustrating the typical experience of an older adult aging in place. We will tackle some of the barriers to this desire shared by over 90% of the population and debunk some of the misunderstandings about what that experience can look like for the typical adult. We will also examine some of the trends and public policy efforts underway to make this dream a reality for more Americans. Attendees will be able to better understand, advocate for, and educate others regarding the practical side of aging in place.

Learning Objectives: 

• Understand the options, risks, costs, and resources available for an American adult to age in place.
• Understand the implications of the decisions and possible barriers to overcome with an aging adult.
• Come away equipped with statistical data associated with the aging population and the ramifications on the overall systems of support.
  

Continuing Education Credits

Each session has been approved for 2 CE credits (NASW Oregon Chapter), AFH training credits, and DHS Administrator credits - that's a total of 8 CE credits if you attend the entire series. There will be a $10 fee per session for NASW credits. Regular Certificates of Attendance can also be issued.

To receive a certificate, attendees must attend the session(s) for their entire length and complete a post-session evaluation survey.

Ab Brody, PhD, RN, FAAN, FPCN is the Mathy Mezey Professor of Geriatric Nursing, Professor of Medicine and Associate Director of the Hartford Institute for Geriatric Nursing at New York University. His work is developing the science of equity-focused pragmatic trials, focused primarily on implementation of evidence-based practices in geriatrics and palliative care in the real world. He practices on the Langone geriatrics and palliative care services and holds leadership roles on the NIA IMPACT Collaboratory and NYU-HHC Clinical Translational Science Institute, amongst others. He was a member of the evaluation team funded by the John A Hartford Foundation for the original Hospital at Home CMS Demonstration Project, where he focused on implementation evaluation, including the clinician roles and functions and facilitators and barriers to implementation of the Hospital at Home model.

Amanda Goodenow, MS, OTR/L joined Colorado Visiting Nurse Association in 2011 as a staff occupational therapist. In 2017 she took on a leadership role as the CAPABLE (Community Aging in Place-Advancing Better Living for Elders) Program Manager, overseeing the program’s clinical operations, including a team of three RNs and two OTs, Habitat for Humanity of Metro Denver partnership, evaluation, and outreach and visibility strategies, and helping to design the growth strategy. She joined the Johns Hopkins School of Nursing CAPABLE team in 2020 as a consultant and now assists the CAPABLE National Center with strategic partnerships to spread the program throughout the U.S. Amanda began her practice 18 years ago in acute care in Topeka, Kansas, following a master's degree from University of Central Arkansas.

Jeff Wiberg is the CEO of Family Resource Home Care and oversees the strategy, culture, and vision for the company’s 20+ locations across Washington, Oregon, and Idaho. Jeff is nationally recognized as a thought leader in the home care industry and utilizes his extensive experience and knowledge in the healthcare continuum to advocate ardently on its behalf. Prior to his current role, Jeff spent 10 years working for Family Home Care in Spokane, WA, where he was ultimately named President and CEO and went on to acquire the firm’s private duty business. In 2018, Jeff led a merger with Seattle-based Family Resource Home Care and was named CEO. He has over 20 years of business leadership experience in industries ranging from hospitals and health systems to human resources consulting and holds a Bachelor of Science in Business Administration from Central Washington University. Jeff is an active member, Past President, and current Board member for the Washington Home Care Association and currently serves as the Board President for the Home Care Association of America. Jeff speaks fluent Spanish, enjoys gourmet cooking, plays the piano, and loves spending quality time with his wife and 5 children.
